Improving lives in Essex

The Essex Success Regime has a drive to improve the lives of 1.2million people in mid and south Essex and to create efficiency across the health and care system in the region.

To support our partners in Essex in achieving their collective aims, UCLPartners has been commissioned to develop and deliver a programme of work focused on local health and care, that will help to improve health outcomes for local patients and the population.

The Essex Success Regime has three areas of focus. In summary these are:

• Improving local services – through bringing together GP, community, mental health and social care providers; offering care at home and; providing more services in the community
• Improving health and wellbeing – through providing more support to stay healthy, joining up care records, preventing illness and disease, earlier diagnosis and better care at the end of life
• Improving care in hospital – bringing together the three hospitals as one group, creating opportunities for centres of excellence.

UCLPartners is bringing its expertise from facilitating successful partnership change programmes to support work in each of these three areas. Supported by Professor Mike Roberts, UCLPartners Lead for Population Health; Professor Martin Marshall, Programme Director for Primary Care, and Charlotte Williams, UCLPartners Chief of Staff; and working closely with the partners in Essex, UCLPartners is supporting the following work:

• GP engagement – building on the success that has been achieved to date by the partners, we will support the development of localities in partnership with the local GP community, and to enable primary care colleagues to scope and co-design the emerging shape of care and support for people outside of hospital.
• Developing models of community provision and care local to home – sharing best practice and learning from other areas to create practical steps that will support the successful implementation of locality models in mid and south Essex.
• Redesigning clinical pathways – using approaches created in frailty and end of life care and supporting a shift towards preventing disease and moving care out of hospital. Linking this to innovative plans coming out of hospital proposals, such as a new approach to chronic obstructive pulmonary disease (COPD).
