Transformation – redesigning care pathways and operating models for lasting change
Healthcare transformation is complex and demanding. UCLPartners provides the expertise, neutrality and ecosystem connections to make it succeed.
Across the country, healthcare providers are managing unprecedented demand and pressure to meet financial and constitutional targets, with limited capacity to spare.
For healthcare providers and commissioners, UCLPartners’ transformation service exists to lead large-scale transformation and deliver sustainable, measurable change. When this is done well, the rewards are significant: reduced fragmentation, better patient outcomes and a more sustainable system.
Who this is for
Health and care providers. This includes acute, community, primary care, mental health and specialist providers. The Transformation team deliver on a range of commissions.
We act as the transformation arm of the North Central London Health Alliance and also deliver specific commissions to support system change for organisations that range from charities to trusts and ICBs. The service is particularly relevant to Directors of Strategy and Transformation, Chief Operating Officers and Clinical Directors responsible for leading or commissioning significant service change. UCLPartners can also provide transformation support to other external partners on a commissioned basis.
How it works
Problem definition
UCLPartners works with you to develop a clear, evidence-based problem statement, drawing on clinical, operational and patient insights to define the challenge and identify solutions.
Pathway & model redesign
Drawing on expertise in end-to-end pathway redesign, digital transformation and data-driven development, UCLPartners works alongside teams to design new models of care and system approaches.
Stakeholder engagement
We facilitate clinical, operational and patient engagement throughout, ensuring that the people most affected by change are involved in shaping it.
Implementation
UCLPartners works directly with partner organisations to deliver transformation. This includes providing hands-on support and programme management, using proven methodologies, such as Quality Improvement and Learning Health Systems, to rapidly capture data and adapt approaches as work progresses. Sharing real-time insight, we keep delivery on track.
Embed change
We support organisations to embed new ways of working so that improvements last, rather than reverting to previous models once the programme concludes.

Is this right for you?
This service is best suited to organisations that are facing a significant transformation challenge that cannot be addressed within a single organisation.
To get the most from the engagement, your organisation needs the internal capacity to support the programme and the commitment to engage clinical and operational teams throughout. Funding to deploy solutions and cross-organisational buy-in are also important prerequisites.
If you are outside this geographical area and interested in transformation support, UCLPartners may be able to help on a commissioned basis;
