Care home resources
We’re bringing together guides and useful resources, from a range of trusted sources, in one place, as an easy-access resource library that can be used by staff working in care homes.
Assessing deterioration
Significant Care
Significant Care is a free paper based tool designed to help carers spot the early signs of deterioration in the health of those they care for and to make good decisions about what to do to help. Here you can find downloads of the tool:
- Booklet for professional printing
- Booklet for landscape printing
- Weekly chart & SBAR (to note observations and actions taken)
- Checklist poster (it can be laminated and placed in a resident / patient room to serve as a reminder of needed checks).
If you have had a chance to review and /or use the resource, we would be very pleased to hear your feedback and appreciate your time in filling in a short survey.
Watch Geraldine Rodgers, Significant Care Lead, describe the benefits of this tool and how to use it.

Supporting relatives
North London Partners and UCLPartners developed a FAQ leaflet to answer some of the common questions relatives have about family members in a care home during the COVID-19 pandemic.
This FAQ leaflet was produced with some of our local care homes and relatives, quality leads, GPs and the Relatives and Residents Association.
