Examples of our work includes:
- Proactive Care Frameworks: We built our Proactive Care Frameworks to support primary care teams in their management of patients with cardiovascular and respiratory long-term conditions. The frameworks are based on new pathway development, virtual consultations, digital solutions and optimal use of the wider primary care team.Â
- Climate Collaborative: Our climate programme will complement local work to deliver the NHS Green Plan, uniting NHS organisations in North Central and North East London and Mid and South Essex to reduce healthcare carbon emissions and tackle the climate emergency.
- Supporting the NHS to adopt new products and services: We help NHS organisations in our region to identify and implement clinically proven, cost-effective products and services that are ready for NHS adoption and spread.
