Using AI to reach people before they hit crisis point

Long-term conditions drive over 70% of inpatient bed days, and more than 15 million people in England live with at least one long-term condition, adding sustained pressure to A&E admissions.

The AI for Urgent and Emergency Care programme, an NHS England-supported collaboration between UCLPartners, NHS North East London Integrated Care Board and Health Navigator, uses AI to identify people at higher risk of unplanned emergency care. They are then offered personalised, phone-based clinical coaching to prevent crises before they occur.

Launched in 2025, this three-year programme expanded significantly during the reporting year, scaling from three boroughs to all seven across North East London.

More than half of the patients contacted accepted the offer of coaching, showing that people value proactive support when it is offered in the right way.

The programme is on track to reach more than 11,000 patients by March 2027.

Patients have shared testimonials describing the coaching as life-changing, helping them understand their health, build confidence and make changes that could reduce their need for urgent care.
