How to implement the UCLPartners Proactive Care Frameworks

A 60-minute online workshop for people based in primary care interested in using the UCLPartners Proactive Care Frameworks.

Join us for an interactive, informative online workshop for all health and care colleagues involved in primary care transformation. The UCLPartners clinical and education teams will explain what the Proactive Care Frameworks are and how to make the most of the Implementation workbook to start using the Frameworks effectively to improve patient care today.

There will be time for attendees to share their experiences of providing long-term condition care, ask questions and discuss the resources with the UCLPartners workshop facilitators.

This 60-minute workshop is repeated each month. The content will be the same so please only book one workshop date.

What are the aims of this workshop?

  • To demonstrate what the UCLPartners Proactive Care Frameworks are
  • To outline how to implement the Frameworks using the workbooks
  • To encourage discussion and questions on how to benefit from these resources

Who should attend?

Attendees are likely to be based in primary care and involved in the transformation of long-term conditions, for instance:

  • Working with patients in primary care (e.g. practice, Primary Care Network, community pharmacy staff)
  • Supporting front-line staff with the delivery of services (e.g. Integrated Care Board, Public Health, Academic Health Science Network, and NHS England)

Attendees from across the United Kingdom in all roles are welcome, including clinical, administration, and management.

What are the UCLPartners Proactive Care Frameworks?

The UCLPartners Proactive Care Frameworks is a comprehensive package of free resources that enable primary care teams to employ a practical solution to long-term condition management. There is currently a Framework for seven conditions; severe mental illness, asthma, COPD, atrial fibrillation, hypertension, type 2 diabetes and lipid management including familial hypercholesterolaemia. The Frameworks are endorsed by the Royal Society of General Practice and featured in two national programmes commissioned by NHS England. Using the Frameworks helps practices and Primary Care Networks to provide robust, patient-centered care while meeting service requirements efficiently using the whole primary care team.

Implementation workbook:

There are four modules within the workbook:

  1. What are the Frameworks?
  2. Risk stratification
  3. Team roles
  4. Taking a QI approach

Each module takes approximately 1 hour to complete and is packed with introductions to key concepts, practical examples of how other primary care teams have benefited from the Frameworks, and tasks for the user to complete independently and with their team.
