How to implement the UCLPartners Proactive Care Frameworks
Join us for an interactive, informative online workshop for all health and care colleagues involved in primary care transformation. The UCLPartners clinical and education teams will explain what the Proactive Care Frameworks are and how to make the most of the Implementation workbook to start using the Frameworks effectively to improve patient care today.
There will be time for attendees to share their experiences of providing long-term condition care, ask questions and discuss the resources with the UCLPartners workshop facilitators.
This 60-minute workshop is repeated each month. The content will be the same so please only book one workshop date.
What are the aims of this workshop?
- To demonstrate what the UCLPartners Proactive Care Frameworks are
- To outline how to implement the Frameworks using the workbooks
- To encourage discussion and questions on how to benefit from these resources
Who should attend?
Attendees are likely to be based in primary care and involved in the transformation of long-term conditions, for instance:
- Working with patients in primary care (e.g. practice, Primary Care Network, community pharmacy staff)
- Supporting front-line staff with the delivery of services (e.g. Integrated Care Board, Public Health, Academic Health Science Network, and NHS England)
Attendees from across the United Kingdom in all roles are welcome, including clinical, administration, and management.
