Using AI-guided Clinical Coaching to improve health across North London
Escalating pressure on urgent and emergency care is well reported, with increased levels of trolley waits and ambulance handover delays. These concerns require a more proactive, preventative approach to reduce demand on the system and support patients to access the right healthcare in the right place, using AI-guided Clinical Coaching.
This new programme of work takes a proactive approach to healthcare by using advanced AI screening technology to identify patients at high or rising risk of unplanned emergency care. These individuals are then offered targeted, phone-based clinical coaching by trained healthcare professionals. This personalised support equips patients with the tools to better manage their conditions and avoid crises, with a specific focus on long term conditions.
This initiative builds on the success of Health Navigator’s large-scale randomised controlled trial in Staffordshire, which demonstrated:
- A 46% reduction in deaths among men over 75.
- A 34% reduction in emergency attendances.
- A 25% reduction in bed days.
- A 26% reduction in GP referrals to secondary care.
This work is funded by NHS England and delivered in partnership with North East London Health and Care Partnership and Health Navigator (HN). The programme will be accompanied by an academic evaluation from LCP Health Analytics and the THIS Institute, Cambridge, to generate the evidence to support further spread and inform future policy for effective proactive care.
Next steps
The programme’s initial rollout will take place in Waltham Forest and Havering through Barts Health NHS Trust and Barking, Havering and Redbridge University Hospitals NHS Trust. Over the coming months, the initiative will be expanded across North East London, reaching thousands of patients in need of support.
We anticipate a significant projected impact for this work and over the three years we are expecting to see:
- 26,673 unplanned bed days saved across North East London.
- Approximately 13,000 fewer A&E attendances annually.
- Improved patient well-being through earlier intervention and better self-management of conditions.
