End of life care
**As of the 31st March 2020, the End of Life Care programme has come to an end but all resources are still available to download. We thank our partners for collaborating and contributing on our educational programmes and look forward to seeing how the field will improve in the future.**
This programme aims to improve the care that patients and those important to them experience in the last weeks and days of life.
Accompanying resources support facilitators to deliver teaching sessions and activities on:
- Recognising gradual and sudden deterioration
- Asking about a resident’s wishes and making a plan of care
- Prompts to review before calling 999
- Supporting loved ones and involving them with care
Dealing with the death of a resident
We designed What’s Best For Lily? to support care homes across London to deliver excellent end of life care for residents.
This follows a successful implementation pilot of the educational materials conducted with care homes in Camden and Enfield using a champion trainer model.
Training for care home staff
We are working with Medicus Health Partners, a GP Consortium in Enfield, to improve EOLC in specific care homes in their remit. Medicus are providing bespoke training to four care homes in Enfield to educate staff to improve practice and EOLC.
