Meet our 2025 CVD Academy Ambassadors

We welcome our 2025 CVD Academy Ambassadors from across North East and North Central London, and Mid and South Essex. Find out more about each Ambassador below.

North East London ICS

Sabina Yasmin

Sabina is an Advanced Pharmacist at a GP practice in London. She specialises in the management of long-term conditions; delivering clinical expertise grounded in evidence-based medicine. Alongside her role, she also provides teaching and development support for local PCN pharmacists. She has previously held roles as a PCN pharmacist, community pharmacist and clinical lead for the COVID-19 Vaccination Programme.

Aniqa Tahsin

Aniqa is a PCN Clinical Pharmacist and Independent Prescriber working for Hackney Marshes PCN. She has a strong foundation in clinical pharmacy having also worked as a rotational hospital pharmacist across various specialities. She has a passion for improving health outcomes, reducing health inequalities and a special interest in cardiovascular disease prevention.

Harjit Singh

Harjit is a principle GP Partner with over 30 years experience. He hopes to inspire his colleagues to participate in
health improvement programs to deliver improved outcomes. His main interests are cardiology and diabetes where he often handles complex cases.

Majid Husein

Majid is a clinical pharmacist with a background in community pharmacy. He has experience of patient counselling, prescription management and community health promotion. He is passionate about providing the best, comprehensive care he can for his patients.

Manpreet Saini

Manpreet has been a qualified pharmacist since 2004 and has experience taking care of community services such as care homes and the urgent care centre. He was one of the first groups of pharmacists who were trained by CPPE to start working in a GP practice. He has recently completed his advanced clinical practice course.

North Central London ICS

Saloni Kapoor

Saloni is a GP Partner and GP Lead at the Rapid Diagnostic Centre in North Middlesex Hospital. She is passionate about the role of lifestyle medicine in primary care and has a diploma in lifestyle medicine from the British Society of Lifestyle Medicine and the International Board of Lifestyle Medicine – as well as being a board-certified lifestyle medicine expert. She would like to see more holistic and personalised healthcare solutions in primary care.

Reena Mandalia

Reena is a clinical pharmacist with over 8 years of experience in general practice. Her role involves conducting structured medication reviews, managing long-term conditions, overseeing DOAC monitoring and operating an INR clinic for warfarin management. She has experience as a Clinical Commissioning Group pharmacist, auditing care homes to ensure high standards of medication safety and compliance.

Chanel Opoku-Acheampong

Chanel is a senior clinical pharmacist working in primary care within a PCN. She is an independent prescriber, specialising in heart failure with reduced ejection fraction. She has been a part of various multidisciplinary teams aiming to provide high quality evidence based care.

Rose MacDonald

Rose is a GP Partner helping to serve an inner-city population of 12,000 patients. She holds a degree in the History of Medicine from UCL and a medical degree from Royal Free and University College Medical School. She is also a member of the Royal College of General Practitioners. Her main areas of clinical interest are women’s health, mental health, metabolic health and longevity and she is a member of the Primary Care Cardiovascular Society.

Ogechukwu Ilozue

Oge is a GP Partner & Trainer and also works as an LMC chair and as a clinical advisor with the NHS England – London region vaccination programme. Cardiovascular health has been one of her interests since her medical junior doctor years.

Malinga Ratwatte

Malinga is a GP with over 8 years clinical practice in the NHS. He began his medical career with Foundation training rotations in East Anglia and earned a Postgraduate Certificate in Medical Education. He later became a Specialty Doctor in Emergency Medicine before completing his GP specialty training in Barnet. Malinga is particularly focused on the role of cholesterol and ethnicity in cardiovascular risk.

Maaria Hasan

Maaria is a PCN clinical pharmacist working in the Haringey GP Federation. Her current role involves managing patients with long-term conditions by optimising medication management, reducing inappropriate prescribing and educating patients on how to manage their medicines. She is also the lead pharmacist for patient safety in her practice.

Mid and South Essex ICS

Jakeline Rodrigues

Jakeline is a Senior Practice Nurse, specialising in Gynaecology and Uro-Gynaecology services. For the last 11 years, she has worked in primary care and taken sub-specialism into chronic diseases. She has completed several university courses including Emergency Gynaecology at King’s Cross University, Urodynamics Study at Bristol University and Primary Care at City University.

Lionah Musengezi

Lionah is a Clinical Team Leader/Advanced Nurse Practitioner and currently works in a PCN providing clinical leadership whilst balancing a clinical workload. She also leads on clinical projects to help tackle health inequalities. She has worked with a charity organisation to promote health and disease prevention.

Brinda Vyas

Brinda is a Medicines Optimisation Lead Clinical Pharmacist and has been working in the primary care setting for three years. She has just finished the PCPEP pathway and her prescribing qualification where her scope was hypertension. She has been a qualified pharmacist for 10 years with a background in community pharmacy.

Christiana Lewis

Christiana is a Lead PCN Pharmacist with over a decade of experience. Drawing on her community pharmacy background, she brings a patient-centred approach, addressing diverse needs such as health literacy and language barriers. She has a particular interest in cardiovascular health and has implemented innovative initiatives, including hypertension clinics, to optimise patient care in her area.

Hayley Reeves

Hayley is a Clinical Pharmacist and currently leads on medicines optimisation projects within the PCN, working to enhance the quality and safety of medication use. She has seven years of experience in hospital pharmacy, where she specialised in antimicrobials.

Alfreda Mafe

Alfreda is an Advanced Clinical Pharmacist Practitioner with over 15 years experience across general practice, community pharmacy, and hospital settings. She has a background in cardiovascular, rheumatoid arthritis and respiratory medicine, complemented by expertise in clinical system infrastructure and organisational management.
