Join UCLPartners and the British Heart Foundation to learn more about practical tools and resources helping primary care teams proactively support patients to manage conditions including hypertension and cholesterol.

About this Event

In this webinar we will focus on the tools and resources available from UCLPartners and the British Heart Foundation to help primary care teams restore routine care for patients at high-risk with cardiovascular conditions during and post-COVID. Resources shared will include UCLPartners’ proactive care frameworks based on new pathway development, virtual consultations, digital solutions and optimal use of the wider primary care team.

Speakers will include UCLPartners’ Helen Williams, National Specialty Adviser for CVD Prevention, and primary care team members who will share their experiences using the frameworks. Further speakers soon to be confirmed.
