Supporting CVD Prevention in Primary Care

Join UCLPartners and the British Heart Foundation to learn more about the resources available to help primary care teams support patients to prevent CVD, using a virtual first approach.

About this Event

  • Why does the way we support CVD prevention need to change in light of the pandemic?
  • What tools and resources are available to help teams support those with hypertension, AF and high cholesterol?
  • How can new ways of working be implemented and what support is available to help make change?

In this webinar we will focus on the tools and resources available from UCLPartners and the British Heart Foundation to help primary care teams to support CVD prevention in those at risk, using a virtual first approach. Resources shared will include UCLPartners’ long-term condition frameworks that enable primary care teams to continue to effectively manage patients in the current climate. 

We will be joined by Dr Matt Kearney, Director for CVD and Primary Care Innovation at UCLPartners and former NHS England National Clinical Director for CVD prevention, who will walk through the case for change in CVD prevention. Guest speakers will include primary care clinicians who will share examples of changes they have made to practice since the start of the pandemic, how they have implemented the changes and what lessons there are for others looking to take a similar approach.

The session will be held on Zoom. You will require a link to join, which you’ll be given upon registration.
