Proactive care @home is a national programme providing a systematic framework for the transformation of long term condition management in primary care.
The COVID-19 pandemic has disrupted care for people with long term conditions such as CVD, hypertension, diabetes, COPD and asthma, and resulted in a universal shift towards remote care. This programme, part of NHS @home, will support people to manage their long term conditions, reduce deterioration/exacerbation and avoid demand for urgent and emergency care.
Proactive Care @home is built around the UCLPartners Proactive Care Frameworks that adopt a population health management approach with stratification and prioritisation, and mobilisation of the wider primary care workforce to better personalise and optimise care.
This two part webinar will provide an introduction to Proactive Care @home, some insights into the learnings from sites that have already implemented the frameworks, and offer the opportunity to learn about how they work in practice.
Programme
Session 1 – Introduction to Proactive care @home, 11.15am – 12.05pm
Open to all. This session will cover a general overview of Proactive Care @home, experience of at least one of our early adopter sites and a panel session Q&A.
Session 2 –– Wave 2 site induction, 12.15pm – 1pm
Open to wave 2 sites only. This session will cover overview of adoption, UCLP – Primrose approach, minimum requirements, implementation support offer and next steps.
Speakers include: Matt Kearney, Programme Director for CVD Prevention and Proactive Care, UCLPartners, Tim Straughan, Director of NHS @home, NHS England and NHS Improvement and a representative from at least one of our early adopter sites.
