Building capability to bring integrated care into reality
Delivering patient-centred integrated care has been a long-term goal for the NHS. Essential to this aim is effective collaboration between multi-disciplinary teams to ensure patients benefit from a holistic approach that maximises the professional expertise of the people involved in their care.
To help achieve this UCLPartners is collaborating with The Dartmouth Institute to deliver a Place-Based Care Network Programme for North Central London (NCL) & North East London (NEL) STPs. It is targeted at those working within STPs who are leading the development of locality based integrated models of working, for example the Care Closer to Home programme in NCL, the Primary Care Home model in NEL and the NCL Local Maternity System Transformation Programme. The programme aims to increase participants’ capabilities and confidence to lead and operationalise these new models within their local organisations making use of well-developed tools and resources discussed in the programme.
Eight multi-discipline, multi-sector teams in total from both NCL and NEL STPs are currently taking part in the programme, with each team made up of at least 6 members comprising a GP, commissioner, manager, local authority leader, social care provider, acute trust clinician and other key stakeholders who are part of the core delivery team for the local integrated care model
The programme has been tailored to suit the local requirements of the participating organisations linked to STP and Five Year Forward view priorities with each team participating in 8 action-led workshops.
The teams have been learning about eight essential capabilities to delivering sustainable and transformative change and using the five design principles for high integrity health systems to design and implement their local integrated care model. Throughout the whole process participants are adapting the learning to their local integrated care programme.
By teams going through the programme together they are able to benefit from being part of a learning network of peers who are going through the same process in the STP, in addition to The Dartmouth Institute’s experience gained by working with the New Care Models team at NHS England and further afield in US based accountable care systems
