How can we transform dementia care with AI?
Dementia is one of the most pressing and complex challenges facing healthcare today. In the UK alone, nearly 1 million people are living with dementia, a figure projected to rise to 1.4 million by 2040.
Despite advances in research, too many people face delayed diagnoses, disjointed care, and limited support after diagnosis. The result is a care pathway that can feel confusing, isolating, and inefficient for everyone involved.
That’s why we are working with Punto Health. Their team have developed the first AI-based integrated platform for cognitive health, combining early detection, daily support, and clinical insights to keep patients, families, and clinicians connected and empowered throughout the care journey.
Barriers to coordinated proactive care
Once a diagnosis is made, clinicians can often face barriers to delivering the best possible care due to siloed information across services. This leads to reactive care and missed opportunities for early intervention.
PuntoClinic is designed to change that. It provides a dashboard for clinicians that integrates data from PuntoTest, a speech-based biomarker for cognitive assessment, along with a continuous symptom registry. This enables remote monitoring, improves care coordination, and supports truly data-driven decision-making. By giving healthcare professionals the tools to understand each patient’s journey more holistically, PuntoClinic supports more efficient and effective care planning.
Fragmented post-diagnostic support
Once diagnosed, many people face a postcode lottery of support that can be fragmented, inconsistent or entirely absent in their area. This leaves many people with dementia and their carers without the guidance they need to manage daily life, making people overwhelmed and isolated.
PuntoCare directly addresses this by delivering personalised support and care plans tailored to individual needs, including cognitive stimulation, physical activity and healthy habits. Families stay informed and engaged, through clinically validated content that interacts with an AI assistant, offering practical tools and guidance to manage daily challenges and maintain continuity of care.
What’s next?
Punto’s vision is a world where everyone can live a vibrant, fulfilling life. We are helping make this a reality in our area by setting up a 10-week pilot of PuntoCare with Enfield Memory Service and North London NHS Trust.
We’re excited to trial innovative apps like PuntoCare and PuntoClinic to make dementia care more accessible, improve understanding for patients and carers, and explore new ways of tracking symptoms and supporting memory clinic reviews. I have been really impressed with the cognitive games, information resources about dementia and the symptoms tracker, so I am excited to get the trial started soon.
