Significant Care

Significant Care is a free paper based tool designed to help carers spot the early signs of deterioration in the health of those they care for and to make good decisions about what to do to help. Here you can find downloads of the tool:

• Booklet for professional printing
• Booklet for landscape printing
• Weekly chart & SBAR (to note observations and actions taken)
• Checklist poster (it can be laminated and placed in a resident / patient room to serve as a reminder of needed checks).

Is your resident becoming unwell?

This resource was delivered by UCLPartners patient safety collaborative in partnership with North London Partners. Part of the National Patient Safety Improvement Programme led by NHS England and NHS Improvement. It is designed for care home staff to remind them and resident’s family members, of the steps needed to take if the resident is deteriorating.

This NHS guide for care home staff and carers provides guidance on recognising when a resident is becoming unwell.

The “Is my resident well” booklet was produced by the North West London Collaboration of Clinical Commissioning Groups (CCGs) with the North West London health and care partnership with associated training materials. The associated (copyrighted) training materials were produced by Dr Asha Katwa for the double HSJ Value Award winning Harrow Planning And Caring Together project (HEE North London funded Partnership in Innovative Education Project) co-designed with St Luke’s Hospice.

North London Partners and UCLPartners developed a FAQ leaflet to answer some of the common questions relatives have about family members in a care home during the COVID-19 pandemic.

This FAQ leaflet was produced with some of our local care homes and relatives, quality leads, GPs and the Relatives and Residents Association.
