The programme
COVID-19 has placed unprecedented pressure on our health system. This brings an added risk to people with long term conditions, such as hypertension, atrial fibrillation and high cholesterol, who need ongoing proactive care to stay well and avoid deterioration. Interruptions to routine care may worsen outcomes for patients, increase their COVID risk and result in exacerbations that further increase pressure on the NHS. As primary care transforms its models of care in response to the pandemic, UCLPartners has developed real world frameworks to support long term condition management.
These frameworks along with a comprehensive implementation support package constitute UCLPartners’ Proactive Care Programme. National interest and demand for the frameworks have resulted in NHSE/I funding to accelerate implementation of the frameworks in four pilot Integrated Care Systems/Clinical Commissioning Groups in England. Further ICSs are expected to join the national programme in financial year 2021/22. Two of the four pilot sites are North Central London and North East London CCGs which are within the UCLP geography.
The role
The Primary Care Lead Nurse role is a 0.6 WTE opportunity to provide expert clinical support on the delivery of our Proactive Care Programme in North East London. Working closely within the Primary Care Network across North East London, the Primary Care Lead Nurse will have both delivery and coaching responsibilities to increase capacity and capability of local clinicians across North East London to implement the Proactive Care Frameworks and UCLP-Primrose workstream.
Within UCLP, the role sits in our Primary Care team and the Lead Nurse will work closely with a Senior Project Manager and Programme Director focused on North East London implementation. It’s a varied role and you will work with a wide range of stakeholders, including clinicians (e.g. Nurses, GPs and pharmacists), operational staff (e.g Practice Managers), senior commissioners (ICS and Integrated Care Partnership [ICP] level), patients, academic health science networks (AHSNs) across England, other NHS staff and the third sector.
The person
The Primary Care Lead Nurse will primarily support the delivery of the Primrose Project, and the wider Proactive Care Programme in North East London. This is an opportunity for an experienced clinician to advance their career portfolio by diversifying into project leadership and cross-organisation matrix working. Success in this role could lead to further senior project management, training and leadership roles within our extensive partnership network.
