Education and training for managing low risk patients

These materials and resources are designed for Health Care Assistants (HCA) or those in suitable roles within the primary care team (this could be pharmacy technician, physician associate etc), to support the management of patients with long-term conditions who are identified as low risk.


Here, you will find protocols that can be used by HCAs or similar, to contact patients with asthma, COPD, type 2 diabetes and cardiovascular disease (coming soon), who are low risk.

We advise these protocols are not used with patients until HCAs or those in similar roles have completed the training and education support that goes with them.

Smoking cessation
The national centre for smoking cessation has developed training to support people to give up smoking. This training takes approximately 30 minutes, it can be accessed here.

Community of Practice events

We currently offer a monthly series of Community of Practice (CoP) events for Primary Care staff. These are held virtually. 

The aim of these is to support staff development by: 

  • Sharing learning and best practice 
  • Collectively problem solving 
  • Celebrating success 
  • Building peer networks 

As local health systems take on the primary care support package, we will set up CoPs based on geography and to build the skills and expertise of different workforce groups, e.g. HCAs, physician associates etc.
