Shifting to prevention in cardiovascular care

Cardiovascular disease (CVD) causes early deaths, health inequalities, and strain on the NHS – yet it is largely preventable.

Created by UCLPartners, CVDACTION helps GPs prevent CVD by combining data tools with patient pathways. It identifies high-risk patients and connects them to the right support, reducing pressure on primary care.

In 10 London demonstrator sites covering 600,000 people, CVDACTION helped GP teams redesign care and improve treatment for blood pressure and cholesterol – two high risk conditions that cause heart attack and stroke.

Across these sites, it has helped 1,785 patients achieve target blood pressure and enabled 426 high-risk patients to start life-saving cholesterol treatment.

Our evaluation and modelling shows that, even in the short time that the programme has run, it is expected to prevent around 40 heart attacks or strokes over five years as a result of these patients receiving optimal treatment. Saving lives and saving the NHS £905,000, with wider positive impacts in social care and reducing the economic cost of lost productivity.
