Improving long-term condition management

Over 26 million people in the UK live with a long-term condition.

This drives 70% of NHS spending and most GP and hospital use.

We are testing new ways to manage care. The approach aims to reduce appointments, improve decision-making, and make services easier to access for people with long-term conditions.

Our Lived Experience Group helping to shape the programme

Since launching a new data-enhanced care pathway across three Primary Care Networks in November 2024, progress has been made. A new algorithm has been built to flag and prioritise patients for review, Data Protection Impact Assessments were secured to ensure safe access to records and people with lived experience of long-term conditions have had a platform to shape the care model around real patient needs.
