Learning from death – Improving care through mortality reviews

This is the first event within the UCLPartners patient safety and mortality review workstream to support learning from hospital deaths.

UCLPartners will bring together partners from across the region to share learning and improve processes and care associated with mortality. The Learning from Deaths workstream is aligned with the National Mortality Case Record Review programme (NMCCR) led by the Royal College of Physicians and Yorkshire and Humber AHSN. It provides a unique opportunity for hospitals to learn from the care that was provided to patients in the time leading up to their death.

We are delighted to welcome Dr Andrew Gibson, Clinical lead of the NMCCR programme. He will introduce the audience to the programme, share insights and reflect on learning and developments across England. Following Andrew, hospitals from the UCLPartners region will share relevant case studies. They will share their experience from undertaking mortality reviews and discuss their key learnings. Afterwards, participants will be able to explore common challenges and identify strategies to accelerate learning from mortality reviews and implement these into practice.
