Supporting General Practice During COVID-19 Webinar

To help keep people healthy and well in their homes, UCLPartners has developed a package of support for primary care to identify, stratify and optimise care in patients with LTCs at high risk of deterioration.

COVID-19 has placed unprecedented pressure on our health system. Rightly so, focus currently must be on supporting patients with or at risk of the virus. However, there are a large cohort of people living with long term conditions (LTCs) that need ongoing, proactive management to prevent a wave of exacerbations post COVID-19.

To help keep people healthy and well in their homes, UCLPartners has developed a package of support for primary care to identify, stratify and optimise care in patients with LTCs at high risk of deterioration. This support package is being designed by primary care clinicians to minimise GP workload by utilising the wider primary care workforce and digital solutions to deliver care in a new way.

The 1 hour webinar will include an:

  • Outline of the UCLP offer for primary care to support the management of LTCs (Asthma, COPD and Diabetes) during COVID-19.
  • Update on the success of the PINCER work.
