AKI Collaborative Framework
The Collaborative Framework document outlines the problem statement, mission, goals and expectations of the UCLPartners AKI Collaborative. The Framework also details the Institute of Healthcare Improvement (IHI) Breakthrough Series (BTS) improvement methodology as well as the AKI Collaborative measurement strategy. This document will assist in providing structure and support for your AKI quality improvement activities.

Improvement requires setting aims. An aim of an improvement project should be time specific, measureable and define a specific population. Achieving agreement on the aim of the project is a key element for maintaining progress throughout the life cycle of the project.
Now that you have the AKI change package that was discussed in learning session 1, this will be a helpful guide for you to establish your local project aims specific to your institution.
This template will assist you with formalising your AKI champions’ team membership and establishing and documenting project aims and measures in addition to any changes and amendment that you may wish to track and document.
This template will assist you with formalising your AKI champions’ team progress through the project over time. This is a great tool to graphically display your team’s progress and track your successes.
This a great diagram that can be used to plan QI activities. It is a conceptual model to help you and your team identify an aim, and then align the relevant system components in order to support the achievement of that aim.
The driver diagram can provide a great way to systematically lay out aspects of your project so that they can be easily discussed, agreed and shares with others. It organises information on proposed activities, in a user-friendly way, so that the relationship between the aim of the project and the changes to be tested and implemented are made clear.
This is a useful tool to carry out small tests of change to support rapid cycles of improvement at an operational level based on the needs of the team and your project aims. It is an effective tool for learning, because all learning derived from testing an idea can be used to inform future PDSA Cycles (whether the outcome is positive or otherwise).
The PDSA template is a useful tool for documenting a small test of change. The PDSA cycle is shorthand for testing a change by developing a plan to test the change (Plan), carrying out the test (Do), observing and learning from the consequences (Study), and determining what modifications should be made to the test (Act).
Use the PDSA template to help your team document your small test of change. Complete a new template for each test of change that you conduct. Keep a file of all your completed PDSA Cycles for future reference so that you can track and evidence your progress.
This worksheet is an extended version of the PDSA Cycle that allows for greater detail to be documented if you feel that the test of change that you are about to conduct needs further content prior to carrying out the PDSA cycle.

Identify and analyse key behaviours relevant to your work, in need of intervention and change using the AKI patient experience feedback form. Designed by Southend University Hospital, the survey will allow the patient to rate various hospital behaviours they experienced during their stay and discharge. Improve the team’s insight on patient experience providing this survey to the patient once discharged.
